ASSUMPTION OF RISK, WAIVER OF LIABILITY, AND INDEMNITY AGREEMENT

In consideration of being permitted to participate in Armageddon laser tag tournament activities, I hereby acknowledge, agree, and represent that I understand the nature of laser tag activities and that I am qualified, in good health, and in proper physical condition to participate in such activities.

ACKNOWLEDGMENT OF RISKS

I acknowledge that laser tag and related tournament activities involve inherent risks including, but not limited to: physical exertion, running, jumping, climbing, potential collisions with other participants or equipment, trips and falls, exposure to strobe lights and fog effects, loud noises, and other hazards that may result in minor or serious injury.

ASSUMPTION OF RISK

I knowingly and freely assume all such risks, both known and unknown, even if arising from the negligence of the organizers, facility operators, or others, and assume full responsibility for my participation.

WAIVER AND RELEASE

I, for myself and on behalf of my heirs, assigns, personal representatives, and next of kin, hereby release, indemnify, and hold harmless Armageddon Tournament, its organizers, sponsors, supervisors, participants, venue operators, and their officers, officials, agents, and employees ("Releasees") with respect to any and all injury, disability, death, or loss or damage to person or property, whether arising from the negligence of the Releasees or otherwise, to the fullest extent permitted by law.

MEDICAL AUTHORIZATION

I authorize emergency medical treatment in the event of injury or illness during my participation. I understand that the organizers do not provide medical insurance and I am responsible for my own medical coverage.

PHOTO/VIDEO RELEASE

I grant permission to Armageddon Tournament and its affiliates to use my image, likeness, and voice in photographs, videos, and other media for promotional and educational purposes without compensation.

RULES AND CONDUCT

I agree to abide by all tournament rules, safety guidelines, and instructions provided by tournament officials. I understand that violation of rules may result in removal from the tournament without refund.

ACKNOWLEDGMENT

I have read this agreement, fully understand its terms, understand that I have given up substantial rights by signing it, and have signed it freely and without any inducement or assurance of any nature. I intend my acceptance to be a complete and unconditional release of all liability to the greatest extent allowed by law.
